New Patient Registration

mulbarton_drawing.gifWe welcome patients in the villages of Mulbarton, Swardeston, Wreningham, Ashwellthorpe, Bracon Ash and the surrounding areas. A map is available below. If you are unsure whether you live in our practice area please pop in or telephone us for more information.

We ask that new patients fill in a standard NHS registration form as well as our practice patient questionnaire. These are available for downloading from the above "Registering Online" tab. Please bring the completed forms, one for each family member, to the surgery. Mulbarton is a family practice and we would not normally accept just one member of a household.

In accordance with a Government directive, all patients have been allocated a named doctor at the surgery they are registered with. However, you can still book an appointment to see whichever doctor you prefer, or whoever is available at a time most convenient to you. If you would like to know who your named doctor is, please ask at reception when you next visit. 

PLEASE NOTE: If you are on regular repeat medication, you will need to see one of our doctors BEFORE a prescription can be issued.
