We welcome patients in the villages of Mulbarton, Swardeston, Wreningham, Ashwellthorpe, Bracon Ash and the surrounding areas. A map is available below. If you are unsure whether you live in our practice area please pop in or telephone us for more information.
We ask that new patients fill in a standard NHS registration form as well as our practice patient questionnaire. These are available for downloading from the above "Registering Online" tab. Please bring the completed forms, one for each family member, to the surgery. Mulbarton is a family practice and we would not normally accept just one member of a household.
In accordance with a Government directive, all patients have been allocated a named doctor at the surgery they are registered with. However, you can still book an appointment to see whichever doctor you prefer, or whoever is available at a time most convenient to you. If you would like to know who your named doctor is, please ask at reception when you next visit.
PLEASE NOTE: If you are on regular repeat medication, you will need to see one of our doctors BEFORE a prescription can be issued.

A Patient Guide to GP Services
If you wish to register with the surgery and you live in our practice area then you can fill in a registration form and a patient questionnaire, one for each member of your household.
The questionnaires are helpful because it can take a considerable time for us to receive your medical records. Please make sure that you complete all sections of the form for each person in order that we can process them.
Please bring these to us as soon as possible in order that we can begin the registration process. If you have been on regular medication a repeat slip from your previous GP is also very useful.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
